Our accredited cardiac lab and the Victoria X-ray Ultrasound Imaging Centre are always accepting new patient referrals. Ultrasound results are back to your office within 24 hours.
On-Site Imaging and Cardiology
Family physicians and specialists across Toronto refer patients here for diagnostic imaging and cardiology assessments. Our team books the patient directly and sends the report back to your office. Your staff does not need to chase scheduling.

All three forms are fillable PDFs: type into the form on your computer, print it for signature, and fax the signed form to (416) 335-0036. Appointments cannot be scheduled until a signed form is on file.
